: The global prevalence of mortality and morbidity among type 2 diabetes mellitus patients is on the rise, highlighting the need for effective treatment to halt disease progression. Traditional medicine is attracting greater interest in type 2 diabetes mellitus management due to its proven efficacy and safety. This study aimed to evaluate the efficacy of Ayurveda based Panchakarma therapy and diet modification in reversing type 2 diabetes mellitus. A retrospective, single center, investigator-initiated study was conducted at a clinic in Maharashtra from September 2018 to October 2023. Patients aged 30–70 years diagnosed with type 2 diabetes mellitus according to the American Diabetes Association, regardless of gender were included in the analysis. Follow-up was conducted at 30, 60, and 90 days. Day 1 and day 90 data were compared. A total of 20 patients were analyzed. Glucose tolerance testing revealed mean glucose levels of 106.35 mg/dL after fasting, 166.89 mg/dL 1 hr postprandial, and 116.59mg/dL 2 hrs postprandial. Glycated hemoglobin (HbA1c) levels decreased (day 1: 7.16 ± 0.88%, day 30: 6.36 ± 0.65%, day 60: 6.07 ± 0.59%, and day 90: 5.94 ± 0.28%) at the 90-day follow-up. Ayurveda-based Panchakarma therapy, when combined with diet modification, significantly contributes to the reversal of type 2 diabetes mellitus. The results indicate a marked reduction in HbA1c levels and an improvement in glucose tolerance, suggesting enhanced metabolic control and insulin sensitivity.
